Then 1st Lt. Richard Brazik and Capt. Richard A. Claflin, pilot, were aboard their F-4C (aircraft #64-0848), call sign "Buick 2," as the number two aircraft in a flight of two flying out of Ubon Airfield at 2100 hours to conduct a night armed reconnaissance mission along Route 15.
Over their target, they proceeded to make a rocket attack pass on the NVA truck convoy. During their second rocket firing pass, their aircraft was damaged by its own ordnance exploding directly below the aircraft. They failed to pull out of the dive and impacted the ground in a fireball on the west side of Route 15.
