Last Known Activity A1C Annis was the hydraulics specialist aboard a KC-135 (# 57-1444) that crashed on take off from Kadena AFB, Okinawa. He served with the 919th Air Refueling Squadron, 484th Bombardment Wing when he was deployed to Kadena Air Force Base, Okinawa, Japan, TDY to the 4252nd Strategic Wing, SAC on Kadena. The aircraft took off in a rain storm from Kadena AFB. Apparently, the aircraft failed to attain an adequate speed on takeoff and crashed near Highway 16, killing a motorist, crashing into an ammunition dump. The aircraft was fully loaded with fuel and had a crew of 10 on board, all of whom were lost. It was bound for Eielson AFB, Alaska.
