Captain Ivan J. Campbell, who liked to be called Johnny, was a Nail Forward Air Controller assigned to the 23rd Tactical Support Squadron at Nakhon Phanom RTAFB, Thailand. Captain Campbell (Then 1st Lt) and Richard C. Swift were flying O-2A S/N 68-6859 when the aircraft struck a tree and crashed. His body was recovered.

Capt. Ivan J. Campbell was born in El Paso, Texas. His father is Therman  Leo Campbell and mother, Mary Leeta Tyler. (Source: Texas Birth Index, 1903-1997,, Roll #1943_0002). His father, Therman Campbell, retired as a Chief Warrant Officer from the U.S. Army.
