Major Warren served out of Danag AB. Major Warren was serving his second tour in Vietnam as the leader of the small unit of Fast Fac aircraft.

He and his co-pilot 1st Lt. R. LeBlanc were flying a "Stormy" Fast FAC F-4D (#66-8820) and were shot down during a mission over southern Laos near Ban Kate. Lt. LeBlanc was rescued.
