his crew was one of those from Fairchild AFB, Washington on TDY with the 4133rd Bomb Wing Provisional.

On 10 May 1969, B-52D (#56-0593) belonging to the 509th Bomb Wing crashed in the Pacific Ocean shortly after take-off from Andersen AFB, Guam. (325th Bomb Squadron, 92nd Bomb Wing)

Captain Broadhead left a young wife and child at this death.
