Major Thomas Case was part of a specially selected and trained crew. They trained at Eglin AFB prior to departing for Danang, from which their final mission would take place.

On his last mission he flew C-130E (serial #64-0511) with the 61st Troop Carrier Squadron. They were part of what was known as Operation Carolina Moon. Their mission, to drop a large magnetic mine on the Hom Rong Bridge northeast edge of Thanh Hoa, North Vietnam. His aircraft was shot down during the second attempt to blow the bridge.
