SMSgt. Thomas Yuji Adachi was a crewman aboard an AC-130A Spectre gunship, call sign "Adlib," (# FT54-1625), flying out of Ubon Airbase on a "Commando Hunt" mission (armed reconnaissance) over the Ho Chi Minh Trail, Saravane Province, Laos.
During its attack, it was brought down by AAA from the jungle below.
At the time formal search efforts were terminated, and the crew became MIA.
On 8 November 1995, a group burial of the unidentifiable co-mingled remains of the AC130A Spectre gunship crew was held at Arlington National Cemetery.
