"Colonel Andrews was a member of the 21st Tactical Air Support Squadron, Pleiku Air Base, South Vietnam. On March 4, 1966, he was the pilot of a Cessna Bird Dog Observation Aircraft (O-1G) on a cross country visual reconnaissance South Vietnam. Radio contact was lost. His remains were not recovered. He was promoted to Colonel while missing in action. He served his country during the Korean War and in Vietnam."

Colonel Andrews was a "Baron" Forward Air Controller (FAC) assigned at Pleiku. He was a Major when he was lost flying O-1E (S/N 56-2499) and promoted while he was missing. His body was not recovered, though finally found and identified in 2006. Their remains had been buried by local villagers.

They were on a visual reconnaissance mission to Cheo Reo, an airstrip approximately 60 miles southwest of Qui Nhon He was lost along with his passenger, First Lieutenant John Francis Conlon (an F-4 pilot), after contacting a Special Forces camp and asked to check some campfires seen in the area. Nothing more was heard from the aircraft and it disappeared.
