He was killed when his and another B-52 collided near Fairchild AFB. Different sources have him as captain, major, or lieutenant colonel at the time of his death. A likely scenario is that his active duty grade was O-3 and that he was posthumously promoted to O-4 and also that he had the reserve grade of O-5.
